Removing Confusing Content

in upload control panel; How do i remove URL Text, or Thumbnail Upload? For some people who post, this is really confusing.

is it save to delete content from *.Html file? Could you give me a example what part i can remove without error? Or could you explain why there is a option URL right at the button of upload?
One more think Can i add some table in description. For example:
I would like to crate something like this:
