Show input info after upload

Hi all

Anyone know, it is possible to show filling informaton from fields, after upload photo. On page where uploaded photo show.

For example:

User fill info in fields: Country (USA), Date (12/12/09)
I need to show this info after uloading photo on member.php

Anyone can help me?

No. You are not understand me. I need to show information to users, while his photo show after download. (in this page writing: you photo will be validated and publish after admin check, uder this located downloaded photo and under this photo i need to show user filling information)
