Best way to approach template design?

I want to design a web Free Templates for a friend of this website.But I need some different templates in use and wanted to get your opinion on the best way to approach template design? Which programs should I use? I know quite a bit but still am not knowledgeable enough to use notepad not yet atleast :) so what do you think I should use?

If HTML, good use for: Notepad (edit content). Upload file ASCII (no binary - or broke). ;)
8 steps need when ask question -

- PHP version (ACP - > phpinfo())
- mySQL version (ACP - > phpinfo())
- 4images version
- Post screenshot / URL
- Post code in BB Code (no need full file for code) or post attach file
- It doesn't work. What is say - what is do for no work
- Install MOD ? If so - please say (troubleshooting)
- Read FAQ ? Install Bug fixes ?
