Welcome to 4images forum.
If no folders created when you created a category, then it means the permissions (alt) are not correct, or SAFE MODE is enabled.

I double checked the permissions on the folders and files, and they are correct.
SAFE MODE is not enabled.

I'm also having the same problem now.

Permissions to 'media' and 'thumbnails' is 777, and safe mode is OFF, I even double checked using PHPinfo.

However I am able to create 'categories' in admin. So permissions are in fact correct.
MAX Upload is also set properly

I'm having this same issue. I've uploaded a smaller file and I was able to create a thumbnail from it. The files that arent working are over 1.5MB in size.

How do I correct this?
