Title: It won't let me upload

heres what i get when i try to upload:

No database entry!

I've already tried chaning the includes folder and everything in it to 777 but it still won't work... can anyone help?

I tried that and its still giving the same error. Any suggestions?

Ok that works... i can upload now but now when i go to do the thumbnails i get this error:
