Users Group signup form

Hi, i need the posibility for my registered users to become a contestant in our contest section, i mean, once i'm registered, i can have something in the left menu that says like "Sign up as a contestant", this will bring up a form, and once the fom is filled up, after admin's approval the user is included into a group with especial permissions.

is this possible??
