Why My member can't register!

Hello,
i have a problem. When register member, after agree page, that show the page for type user name, password and email.
But it still have a notic "Please enter the verification code". Memmber can't register success. Please help me resolve it. Thank you for help!

and did they enter the correct verification code from the "captcha" image?

Thank you, i fixed it.
i used template of hotdt.com, so it miss captcha image.
i edit and add code for some page.
