It won't let me upload

heres what i get when i try to upload:

No database entry!

I've already tried chaning the includes folder and everything in it to 777 but it still won't work... can anyone help?

This is folder permission setting problem in FTP - here:

- PHP version (ACP - > phpinfo())
- mySQL version (ACP - > phpinfo())
- 4images version
- Post screenshot / URL
- Post code in BB Code (no need full file for code) or post attach file
- It doesn't work. What is say - what is do for no work
- Install MOD ? If so - please say (troubleshooting)
- Read FAQ ? Install Bug fixes ?

Ok that works... i can upload now but now when i go to do the thumbnails i get this error:
